Coastal Oaks Recovery Located in the heart of the Lowcountry in
South Carolina, Coastal Oaks Recovery is a 72-bed substance-use
treatment center serving adults aged eighteen and older. Nestled on
five serene acres near the coastal waters and wetlands of Port
Royal, Coastal Oaks provides a tranquil environment for individuals
suffering from addiction to stabilize and embark on their journey
toward recovery and wellness. At Coastal Oaks Recovery, we offer
medically monitored detoxification under the direct care of
physicians and nurses, supported by a multidisciplinary treatment
team. In addition to withdrawal management, our residential
treatment services provide a safe and structured environment for 30
to 45 days, ensuring that each patient builds a solid foundation of
recovery skills prior to discharge. Our evidence-based programming
includes group therapy, individual counseling, family programming,
and specialized care tailored to meet individual needs. With a
holistic and experiential approach, we emphasize safety,
collaboration, and a patient-first culture that nurtures recovery
and wellness for both patients and staff. Position Purpose The Case
Manager provides comprehensive case management services to patients
during their treatment at Coastal Oaks Recovery. This role involves
conducting evaluations, formulating individualized care plans,
coordinating discharge planning, and collaborating with a
multidisciplinary team to ensure seamless transitions and
continuity of care. The Case Manager plays a critical role in
advocating for patients and connecting them with community
resources to support sustained recovery. Essential Duties and
Responsibilities Patient Assessment and Planning Through the
direction and scheduling of the Director of Clinical Services, the
Case Manager will facilitate several contact appointments with each
patient assigned to their caseload and conduct thorough assessments
of patients’ internal treatment needs and external support during
and upon discharge from treatment. Develop and implement
individualized case management plans that address treatment goals
defined in the patient’s Master Treatment Plan and discharge
planning needs. Responsible for assisting patients with the
coordination of any short and long-term disability, work-related
absences, and issues during the course of their care. Work directly
with the patient’s primary therapist and multi-disciplinary
treatment team to identify the patient’s vocational, familial,
educational, social, financial, medical, and recovery needs.
Directly schedule appointments and meetings with the patient during
treatment and upon discharge of each patient. Attend weekly
treatment team meetings and provide progress updates for each
patient on your caseload. Coordinate transportation for patients to
and from Coastal Oaks when requested. Assist patients in obtaining
legal and work-related documents from outside sources to ensure a
seamless entry to treatment and re-entry to discharge. Coordinate
referrals to appropriate placements for patients upon discharge
Have a clear understanding of the patient’s date of discharge and
directly communicate with all referral sources to ensure a seamless
transition for patients upon discharge. Collaborate with patients,
families, and the treatment team to create a comprehensive
discharge plan, including referrals to community resources,
housing, and employment support. Collaboration and Advocacy Act as
a liaison between patients, families, and community resources to
ensure continuity of care. Educate patients and families about the
recovery process, including their rights and responsibilities.
Advocate for patients’ needs, including access to medical, legal,
and financial resources. Work closely with the clinical team to
provide holistic, patient-centered care. Discharge Coordination and
Follow-Up Coordinate discharge plans, ensuring all necessary
resources and supports are in place for patients’ transitions.
Arrange follow-up care and aftercare services, including outpatient
treatment and support groups. Monitor and evaluate the
effectiveness of discharge plans, making adjustments as necessary.
Documentation and Compliance Maintain accurate and timely
documentation of all case management activities in accordance with
Coastal Oaks Recovery policies and procedures. Ensure compliance
with HIPAA and confidentiality regulations. (Specific to Section 43
CFR) Regularly update patient records to reflect progress and
changes in treatment plans and report to primary therapist and
Director of Clinical Services. Skills and Abilities Strong
communication, organizational, and interpersonal skills. Ability to
work effectively in a multidisciplinary team environment. Knowledge
of substance use disorders, co-occurring conditions, and
evidence-based treatment approaches. Proficiency in electronic
medical records (EMR) systems. Ability to manage challenging
situations with empathy and sound judgment. Education and
Experience Required : Associate degree in Behavioral Science,
Psychology, Sociology, or a related field. Preferred : Bachelor’s
degree in a related field. Minimum of one year of experience in
case management or counseling within a substance use treatment or
mental health setting. Familiarity with community resources and
discharge planning processes. Licensure/Certification Current CPR
and First Aid certification. HIV/AIDS training (required every two
years). Case Management Certification is preferred. Additional
certifications related to substance use treatment are a plus.
Physical and Work Environment Requirements Frequent standing,
walking, bending, and sitting. Ability to lift up to 20 pounds
occasionally. Work primarily indoors in a temperature-controlled
environment. Ability to maintain professionalism and empathy in
