Grow patient volume in Southeast GA by building relationships
with case managers, physicians and other referral sources.
Hire, onboard and orient a team of caregivers that provide
outstanding care to the patients and families in the market.
Oversee the provision of the care to patients in their homes
consistent with standards of practice, community need, agency
policies and procedures, Medicare/Medicaid guidelines and State
licensure Assures medical records are maintained appropriately and
that all clinical disciplines function according to licensure,
state law, and Medicare/Medicaid standards.
Supervises the provision of clinical nursing care to patients
and provides care on an as needed basis
Works with clinical staff to assist as needed with difficult
care issues and to alter patient plans of care
Be able to conduct patient visits with a strong focus on why ACP
can offer them the best care possible. Work in conjunction with our
Marketing and Business Development team to understand why ACP is a
market leader and what we can offer our patients that no one else
Assist in managing the safety, infection control, Quality
Assurance/Performance Improvement program which includes oversight
of the clinical record review and chart audits and maintenance of
Advises, assists and collaborates with clinicians on an
as-needed basis to assess patient situations, staff problems, and
coordinate physician referrals.
Assists Director of Nursing with completion of all clinical
paperwork related to patient care, for GAPP and COMP renewals.
Conduct monthly supervisory visits and documents appropriately,
according and compliant with state requirements.
Able to be on-call after hours, on weekends within rotation
scheduled and as needed basis.
Identifies staff development needs and ensures that orientation
is held and continuing education opportunities for clinical staff
are sought and encouraged.
Assists Director of Nursing in conducting employee performance
evaluations/appraisals at the completion of the probationary period
and annually thereafter within 30 days of the evaluation due
Assures accurate and timely completion of incident reports in
partnership with Human Resources and performs audits of nursing
notes for compliance.
Assists in the investigation of patient and customer concerns
and complaints, informs the Director of Nursing of patient care
and/or staffing issues or personnel issues on a timely basis.
Assures that an adequate stock of medical supplies needed for
patient care provision is available for use by staff for patient
care within the home.
Assures that home care nurses receive supervision, either
directly or via other appropriate nursing staff.
Maintains current knowledge of home care principles and
practices, applicable laws and regulations.
Attends external meetings as needed and presents information on
Advanced Care Partners Services to outside sources.
Maintains confidentiality of patient and agency information
according to HIPAA guidelines.
Performs other duties as assigned.
Current Georgia or compact state registered nurse licensure
Bachelor’s degree in nursing
Minimum 3 years of experience in nursing, at least on year in
home or community health nursing preferable.
Minimum 1 year in a supervisory or management position
Valid Driver’s License, reliable transportation and car
insurance throughout employment.
Fingerprinting completed in accordance with Medicaid
