highest level of care in our communities SUMMARY Responsible for
effectively assisting referral sources and community resources in
order to facilitate the transition of prospective patients into the
appropriate level of care within the System. Meets with patients
and their families to provide explanations of services available
within the organization's health delivery system. Coordinates
community education and maintains open, effective relationships
with providers, physicians, and community organizations.
Coordinates development of relationships with quality providers as
needed and directed. ESSENTIAL DUTIES AND RESPONSIBILITIES:
Represents the System in the field to facilitate referrals and
admissions. Completes referral and outreach documentation and
obtains organization specific admission documentation when
necessary. Performs additional information gathering related to
prospective patients when requested and communicates with service
line and referral source under the direction of the receiving
member organization. Communicates with organization personnel
regarding patient transitions. Collaborates with member
organization leadership in the establishment and maintenance of
community outreach education plans. Coordinates new and existing
relationship building with all service lines and major and minor
referral sources. Implements outreach programs designed to inform
and educate providers and the community on the services and
programs offered by the organization they are representing.
Establishes and maintains professional relationships with all
referral sources, including but not limited to case managers,
discharge planners, social workers, physicians and physician
practices, and any other community or third party referral sources.
Completes in-person outreach visits with referral sources and
documents visit details in the appropriate system. Responsible for
meeting established goals related to referral relationship
development, outreach program implementation and other quality
metrics established. Performs other duties as assigned by
supervisor. Performs duties in a manner consistent with compliance
and ethical standards of the organization, regulatory and legal
bodies. Promotes the image and reputation of the System by
exhibiting servant leadership and providing direct and open lines
of communication. Contributes to the work of committees,
workgroups, project management, and other collaborative efforts of
the System. Performs other duties as necessary to ensure the
success of the System. SKILLS AND ABILITIES: Thorough knowledge of
licensed skilled nursing facility operations, regulations and
requirements for admission. Thorough knowledge of home health
operations, regulations, and conditions of participation. Thorough
knowledge and ability to perform on site assistance with
information gathering on behalf of the member organizations.
Portrays a positive image of the organization and communicates
guiding principles, mission, vision and values. Ability to work a
demanding, primarily self-directed work schedule. Ability to
deliver excellent customer service, externally and internally as
well as maintain customer confidentiality. MINIMUM QUALIFICATIONS:
Bachelors or associates degree preferred Minimum of 2 years'
experience in marketing healthcare products or services, long-term
care, inpatient rehabilitation or health care community relations.
Valid Georgia Driver's License SUPERVISOR RESPONSIBILITIES None
LANGUAGE SKILLS Ability to read, analyze and interpret the most
complex documents. Ability to respond effectively to the most
sensitive inquiries or complaints. Ability to make effective and
persuasive speeches and presentations on controversial or complex
topics to clients and members of leadership. MATHEMATICAL SKILLS
Ability to add, subtract, multiply and divide in all units of
measure, using whole numbers, common fractions, and decimals.
REASONING ABILITY Ability to apply common sense understanding to
carry out instructions furnished in written, oral or diagram
format. The ability to deal with problems involving several
concrete variables in standardized situations. Possess excellent
critical thinking skills in order to facilitate real-time
decisions. COMPUTER APPLICATION SKILLS This job requires
proficiency with the following computer applications: Microsoft
